About Us

Goals

  • Provide both coordination and access to primary care services throughout Windsor-Essex to Immigrants and Refugees
  • Refer patients to primary care through a clinical connect process to the most appropriate primary care provider based on need, language and geography
  • Facilitate a seamless transfer to a primary care provider after conducting an initial assessment and addressing immediate concerns
  • Create a warm welcome into our health community
  • Provide education on healthy living and wellness in multiple languages.
